ACTIVITIES UPDATE FOR 2023-2024: Bound Registration Required for Participating Students

If your student is interested in participating in any extracurricular activities (FINE ARTS, ATHLETICS OR CLUBS), parents must register each student via Bound.

ALL STUDENTS planning to participate in activities must be registered on Bound. This includes fine arts, athletics and clubs.

If your student is considering participating in athletics or cheer, parents must also upload a Physical, Concussion and Permission/Release Form via Bound. These forms will now be uploaded through this process – paper forms will not be accepted moving forward. You will not need to print a form elsewhere.

If you can not locate a current physical (and it’s currently on file with Xavier HS), please contact the XHS Activities Office via email at [email protected].

If your student is planning on (or interested in) participating in multiple activities, please register for each activity. The following forms will be uploaded/signed electronically via Bound:

• Physical Form (Required for Athletics and Cheer)
• Heads Up Concussion Form (Required for Athletics and Cheer)
• Parent or Guardian Permission and Release Form
